Health Liaison Toolkit

getting started

1. Choose your Contacts

 Decide who you want to support as a Health Liaison. This can be family members, friends, neighbors, or others in your community. Explain your role and ask for their permission before you begin.

2. Set a Schedule

 You will meet once a month. Choose a consistent date and time that works for both of you.

3. Action!

At each meeting, use the Health Liaison Notes Sheet to guide your conversation. The sheet includes simple questions to help check on overall health and well-being.

How to use the Health Liaison notes sheet

1. Meet and Interview

When you meet with your contact, use the Notes Sheet to record information during the interview. Refer to our Resources page as needed (e.g., learn how to calculate BMI). Health Liaisons do not diagnose or provide medical treatment. You do not need to ask sensitive questions. Continue completing each section until you reach the “Finish!” mark. At that point, the meeting is complete.

2. Review and Assess

Once the questionnaire is filled out, review the responses and notes you took for each category together. This will allow them to find areas of concern and think of opportunities for improvement.  Use our Resources page for guidance and support. Remember, you are NOT diagnosing. Your role is to support lifestyle changes that promote better health. 

3. Follow-up and Support

Together, come up with a plan they can work on over the month toward a healthier lifestyle.  At the next monthly meeting, check in to review their progress and provide additional guidance if needed.
